Qualified staff members may wish to purchase equipment using a pro-form.  Please remember that each company sets the requirements regarding who may or may not purchase their equipment on a pro-form.  Some companies will require that you be a PSIA certified level III instructor others will simply require membership in PSIA.  If you do not have a current PSIA card do not bother to contact any of the reps listed below. 

 

Equipment is for your personal use only.  You are expected to represent the equipment in a positive manner.  Failure to follow the guidelines jeopardizes the pro-program for everyone.
